Customer feedback surveys

Hello, we are looking at setting up customer feedback across channels. Has anyone set up telephony surveys that would be willing to discuss their experience of this. What questions you’ve asked, how customers have been approached, success rate of customer response etc

How do you present your surveys to customers are they SMS/ end of call? what questions do you ask? what is your success rate of customers completing these? have you ever changed what you ask? and if it’s ok to ask, what LA are you from?
